![]() ![]() Due to data confidentiality, a random sample of 1447 (CD: 724 UC: 723) members was drawn by the DCCV according to the last digit of the membership number. Materials and Methods SubjectsThe German IBD association (DCCV) is the largest voluntary support organization for IBD patients in Germany, with about 15,000 members including patients, relatives, and physicians. Specifically, the study aimed to evaluate the responsiveness of EQ VAS and EQ-5D index scores to patient reported improvement or worsening of health status in terms of meaningful differences. 27 The purpose of this study was to assess the validity, reliability, and responsiveness of the EQ-5D in a much larger sample of subjects with CD and UC. 26 Although MDs may vary across patients and patient groups, they are useful for comparing individual changes to those perceived to be important in the whole group of patients. The EQ-5D has been previously tested in an IBD patient population. 25 Increasingly, MDs are based primarily on the patient's perspective 25 and are defined as a change perceived by patients to be beneficial or harmful. Quicktime app logo for macA response rate of only 40% was predicted by the DCCV based on past experience with HRQOL studies, 28 which provide no immediate benefits to the members themselves and which must compete with other studies (e.g., drug studies) with more obvious benefits, also recruiting through the DCCV. One reminder letter was also sent after about 6 weeks. Documents were provided in German. The DCCV mailed these members asking them to participate in a postal survey evaluating disease costs and quality of life. Chosen subjects were to be 18 years of age or older and have clearly indicated that they had either CD or UC on their membership. Subjects were asked to specify the type of IBD (CD or UC). Clinical and Demographic VariablesDemographic and medical information was documented by participating members in a questionnaire. The study was approved by the Ethics Committee and patients provided written informed consent. In further analyses, active disease was defined by an activity index greater than 3 for both CD and UC subjects. Missing answers regarding extraintestinal manifestations, fistulas, fever, body mass index, or bloody stools were assumed to have a value equal to normal. If there were missing answers for number of episodes of diarrhea, abdominal pain, or general well-being the index was also not calculated. 31 For both activity indexes, if the patient documented a stoma operation in the past, the activity index was not calculated. 30 Although both activity indices are usually calculated by physicians with access to laboratory and physical exam data, we used only patient-reported data consistent with the methods reported by Janke et al. Separate questions for CD and UC were posed based on the Crohn's Disease Activity Index (CDAI) according to Best et al for CD 29 and the Clinical Activity Index (CAI) according to Rachmilewitz for UC. First, the correlation of EQ VAS scores and EQ-5D index scores with the calculated disease activity indices was determined. Assessment of Validity, Reliability, and ResponsivenessConstruct validity was evaluated in 2 respects. For the EQ-5D index scores in this study, a “social tariff” time trade off (TTO) utility score was calculated based on Greiner et al's weights, 11 reflecting German TTO valuations, and on Dolan et al's weights (UK population) since it is based on a large sample 3 and is well accepted (see introduction). At follow-up, subjects reported whether their health status was better, worse, or the same in a transition question. Subjects received 2 questionnaires and were requested to fill in the EQ-5D descriptive system and EQ VAS upon receiving the questionnaire (baseline) and 4 weeks later (follow-up). Respondents reported their current health profile and rated their perceived overall health on the EQ VAS. The coefficients of better (β b) and worse (β w) in the regression model estimate the incremental difference in scores when health status shifts to better or worse (according to the patient's view) compared to the stable health status. 33 SRM was calculated by dividing the difference of the means at the 2 timepoints by the SD of the differences of scores between the 2 timepoints. The difference in score (DScore) was the dependent variable and the answers to the transition question whether health status was better (X better) or worse (X worse) were entered as dummy variables. ES was calculated by dividing the difference of the means at the 2 timepoints by the standard deviation (SD) of the baseline mean which relates the change to the baseline SD. The simple kappa was used to test for agreement of categorical variables while the Intraclass Correlation Coefficient (ICC), calculated using a 2-way analysis of variance (ANOVA), 32 was used to test agreement between continuous variables.Responsiveness (sensitivity to change) of EQ VAS scores and the index scores was tested in subjects indicating a change in their health status in the transition question with paired t-tests, effect size (ES), and standardized response mean (SRM). These comparisons were performed for CD and UC patients, and subgroups with active and inactive disease.
